Boone, North Carolina
Piper PA-32 N43576 (Small Plane)
Crashed near Boone Golf Club around 1 pm. At least one died, 2 with serious burns. Ben Chappell, 69, of Boone died at the scene of the crash. Two others inside the plane were identified as John Field Worsley II, 68, of Lenoir and Steven Cox Berry, 43, of Banner Elk. The Boone Police Department will be conducting the initial investigation, however the FAA and the National Transportation Safety Board will be responding to conduct a further investigation.
From the preliminary report: “According to a witness at the approach end of runway 31, he stated that the airplane was approaching at a high rate of speed prior to landing. He said that he did not see the airplane touchdown, but shortly after he saw a plume of smoke on a golf course. Another witness that was in a house on a hill on the right side of the departure end of runway 31, stated that when she saw the airplane, the left wing was low and it collided with a pine tree. She said that the airplane seemed as if it was attempting to gain altitude but collided with another pine tree before descending and “crashing into the golf course.” Witnesses on the golf course reported that they watched the airplane climb out; they stated that the airplane was “bobbling” up and down before hitting the top of a stand of pine trees. The airplane nosed down and collided on the golf course before bursting into flames. The witnesses on the golf course observed the occupants exit the airplane and assisted them before the local authorities arrived.”

Surprise, Arizona
Fixed-wing, single-engine 1956 Beech G35 BE35 (Small Plane)
The plane crashed under unknown circumstances around 4:20 p.m., according to Ian Gregor, an FAA spokesman
Plane lost engine power, crashed into a greenbelt in Sun Village, an adult community near Litchfield and Bell roads in Surprise. Joshua Merrill, 31, of Peoria, suffered fatal injuries.
“Investigators determined that the plane had recently undergone maintenance and that the pilot had taken it airborne to check the status of the plane. The pilot used this opportunity to provide instruction to a private pilot who requested a checkout in the airplane, according to the report….The report noted that the pilot, who was said to be a flight instructor who held a mechanic’s certificate, had performed the aircraft maintenance. A witness who spoke with the NTSB told investigators the plane appeared to be “low, slow, and quiet” before it disappeared behind a building.”
One witness Sue Petty, said ““I said, ‘My Lord, what was that? The loudest boom!” It’s unclear if this is when the plane was in the sky or the crash.
Another quote from her, “I came running out of a lady’s house and she didn’t hear it,” Petty said. “But I thought, man, you ought to be deaf if you didn’t hear it.”
“Officials from the National Transportation Safety Board and Federal Aviation Administration are investigating the crash.”

Wantage, New Jersey
6-seat 1966 Cessna plane (Small Plane)
59-year-old Lu Chen died from this crash and was found in a wooded area in Wantage, New Jersey, on May 19.
The plane was found three days after it crashed, upside down with its wings torn off.
“It’s too bad. As a pilot, you read the NTSB report, and there are an awful lot of unanswered questions,” Costa said. “A lot of stuff is not making sense.“
Tupelo, Mississippi
Single-engine private plane (Small Plane)
“Investigators said the accident was reported at 8:34 a.m. near Colonial Estate off Buffalo Road, and first responders were on the scene four minutes later… The plane had just taken off from the Tupelo Regional Airport when the crash took place.”
The FAA is gathering their team of investigators and are headed to the scene.
Four people died Monday when a small plane crashed shortly after taking off from Tupelo Regional Airport in Mississippi. The pilot and three passengers all died when the single-engine plane crashed in a wooded area near a sewage pumping station and the Tupelo Buffalo Park & Zoo. No one on the ground was hurt.
“‘We actually thought it was thunder,’ Carwile said. ‘I was in the gift shop and this man came beating on the door, telling me to open the door. I thought we were being robbed. He was actually trying to tell me the plane had come down and to call 911.’”
“Federal Aviation Administration (FAA) spokeswoman Arlene Salac said the pilot reported smoke in the cockpit of the Beechcraft Bonanza before the crash. She said the FAA will investigate and the National Transportation Safety Board will determine the crash’s cause. Tupelo Police Chief Bart Aguirre told reporters in an afternoon news conference that officials were awaiting the arrival of federal investigators.”

Mesa, Arizona
Single-engine AT-6 Warbird (Small Plane)
“Two people were killed when a fixed-wing airplane crashed as it was taking off from Falcon Field Airport in Mesa Tuesday evening.
Emergency crews were dispatched at about 6:45 p.m. The Mesa Fire and Medical Department confirmed the fatalities less than 30 minutes later.
The single-engine AT-6 burst into flames on impact.
Detective Steve Berry of the Mesa Police Department said the plane experienced a mechanical difficulty. The plane, which had taken off in tandem with another aircraft, crashed as the pilot was turning right.
A witness, however, said that the plane was about to hit another aircraft. According to Martin Cervantez, it looked like the pilot lost control while maneuvering to avoid a collision, causing the plane to cartwheel.
“The AT-6 was flown during World War II and into the 1970s. The warbird, which is often used for both static displays and aerial demonstrations at airshows throughout the country, was the go-to stand-in for various Japanese planes in movies about World War II.”
Berry could not confirm that account, saying only that everything was “very preliminary.” He said the pilot of the other plane circled and landed without incident.”

A pilot who died Wednesday in a small plane crash in Kiowa County has been identified.
John Lee Stubblefield, 64, of Meridian, Idaho, was piloting a Cessna P210 when the plane crashed in a field northeast of Sheridan Lake, about three miles north of Colorado 96 just west of County Road 71, according to a Kiowa County Sheriff’s Office media release.
Two F16 jets and a B1 Bomber training in the area helped emergency responders locate the crash site after the plane vanished from radar near the Kansas and Colorado state line. Stubblefield died in the crash.
The National Transportation Safety Board is investigating the cause of the crash.
Micki Trost, spokeswoman for the Colorado Division of Homeland Security and Emergency Management, said the crash was reported about 9:30 a.m. by a 911 caller.
Allen Kenitzer, spokesman for the Federal Aviation Administration, said the plane was a Cessna 210 and that it went down under “unknown circumstances.” Kenitzer said local authorities reported only one person — the pilot — was aboard.

West Reno, NV
Cessna 182A (Small Plane)
A small plane crashed in West Reno on Tuesday afternoon after its engine failed, according to the Federal Aviation Administration.
“The pilot declined to comment on Tuesday, and Jenn Drew said she had no information about the crash-landing.
The pilot was trying to avoided traffic on Interstate 80 and landed on McCarran Boulevard instead, Battalion Chief Mark Winkleman, of the Reno Fire Department, said Tuesday.
“He looked at I-80, and I-80 was pretty much packed with vehicles,” Winkleman said of the pilot Tuesday. “He didn’t want to use that option, so he found a much less traveled route at the time and put it in a good spot.
“So I think his training paid off and he was able to do a pretty good job of getting the aircraft to where it is now.””

Microlight 2-Seater (Small Plane)
A light aircraft crashed just before 5 PM in a field in Shropshire near Shaw Lane, Shifnal, near Telford, England. The Air Accidents Investigation Branch (AAIB) has been called in to carry out inquiries.
Superintendent Kevin Purcell said: “This is a very sad incident in which two people have tragically lost their lives, we are making sure specially trained officers are making contact with their next of kin and giving them all the support they need.
One onlooker described how the plane, which the AAIB said was a microlight aircraft, was flying normally before it suddenly “nosedived”.
John Taylor, a 37-year-old geography teacher who was watching the skies from a few miles away, said: “I was in the park with my two kids and happened to look up and see the plane.
“I watched it for a few seconds, all seemed normal but then it seemed to be getting lower.
“All of a sudden it nosedived to the ground. It dropped below the roofs of the houses so I didn’t actually see it land.”
The site of the crash was near Shifnal airfield, where the Wrekin Microlight Flying Club is based.
A microlight is a light-weight aircraft designed to hold a maximum of two people and requires a pilot’s license to be flown.
Other eyewitnesses reported that it was a fixed-wing aircraft, which was seen losing power and making a steep descent before crashing into the field.

Witnesses told authorities Warner was performing stunts before the plane malfunctioned and crashed. One of the plane’s wings reportedly bent backwards. The Missouri State Highway Patrol said an investigation into the crash has been launched.
It’s not registered with the FAA, so they aren’t involved in this and we are just going to take the report on it. So it’s non-registered aircraft.”
“He wasn’t doing flips, but he was doing some kind of stunts while flying up there,” said Nothum. “Apparently there was some kind of failure on one of the wings. Something happened to one of the wings while he was flying and that was when he fell or dropped.”

Montana Laurel Airport
Piper Supercub PA 18 (Small Plane)
Federal agencies are investigating a plane crash that occurred Monday morning at Laurel Municipal Airport.
Sgt. Kevin Cunningham said the Yellowstone County Sheriff’s Office received a report about the plane crash at 11:26 a.m. Monday.
Cunningham said the pilot was the only person inside the Piper Super Cub. Cunningham said the pilot is uninjured. The plane appears to be a complete loss.
“The pilot who was relatively uninjured, he didn’t require transport to the hospital or anything like that, reported to us when we spoke with him he had caught a severe crosswind and that’s what caused the accident,” Cunningham said.
The National Transportation Safety Board and the Federal Aviation Administration are investigating the crash.

Single-Engine Plane Crash
“The Goldsmith man killed Wednesday night when he crashed a single-engine plane into a concrete silo had previously been recognized by aviation officials as an experienced pilot, records revealed.
Clayton Ray Chennault, 46, was included in the national Airmen Certification Database in 2013 by the Federal Aviation Administration (FAA).
The database is accessible through the agency’s website and lists certified pilots who have met or exceeded educational, licensing and medical standards established by aviation officials. The standards have “evolved over time” in an attempt to reduce pilot errors that lead to deadly crashes, the agency said.
The cause of Wednesday night’s fatal accident remains undetermined, according to National Transportation Safety Board (NTSB) officials.
“We are in the very early stages of the investigation,” NTSB spokesman Peter Knudson said Thursday. “These investigations are complex. It could take up to a year to 18 months to complete.”
Investigators have not ruled out any contributing factors, he added, including weather…Preliminary NTSB reports listed the time of the accident as approximately 7 p.m., Knudson said. Local authorities reported responding to the crash site around 9 p.m.
FAA records listed the plane Thursday as “damaged beyond repair.”
Witnesses and area residents who saw the plane flying overhead in the moments before it crashed claimed it was hovering notably ‘low to the ground.’
NTSB and FAA officials were unable to confirm the aircraft’s pre-crash altitude, flight path and destination, only revealing it appeared to be traveling eastbound.”
“According to the FAA Registry, the plane is registered to N-Number N5679G which belongs to LM Air Patrol Service Inc. LM Air Patrol Services is based in Midland Texas.”
“The plane crashed by an experienced pilot into the Cemex plant on June 1 was reported stolen to Midland police and investigators are treating the case as a possible suicide, City of Midland officials confirmed Tuesday.
Midland officials have not made the police report public pending a formal open records request but spokeswoman Sara Bustilloz discussed investigators’ findings in a Tuesday interview. She said investigators closed the investigation because the only person suspected of any crime died in the crash.
Police were called at about 9:07 p.m. June 1 to Basin Aviation, a fixed-base operator at Midland Airpark in northeast Midland, 901 Veterans Airpark Lane. Bustilloz said the caller was a woman who said her fiancé might have been in a plane crash.
“She indicated to us that the plane crash could have possibly been a suicide,” Bustilloz said.”

1940’s Aeronca Champ (Small Plane)
“The accident occurred at 8:58 a.m. under ideal flying conditions. The two men inside the two-seater aircraft, a 68-year-old male and a 70-year-old male, were transported to Christiana Hospital in Stanton, Del., according to police. Both are in stable condition with non-life threatening injuries. Their names are not being released until their families are notified.
A preliminary investigation revealed that the aircraft, a 1940’s Aeronca Champ left the runway heading in a northeasterly direction when it lost power and attempted to return to the airfield. Upon attempting the turn, the aircraft collided on the east side of a hangar penetrating into the structure where it came to rest. No persons were present inside and minimal property damage occurred to objects within the hangar. The hangar structure has suffered damage and is being evaluated by township engineers.
John Martin, manager of New Garden Flying Field, was unavailable for comment. Officials from the Federal Aviation Administration were called to the scene.”

F/A-18 Hornet Blue Angel (Small Plane)
An F/A-18 Hornet with the Navy’s elite demonstration squadron crashed shortly after take-off Thursday, killing the pilot just days before a weekend air show performance, officials said.
A U.S. official told the Associated Press that the pilot was Marine Capt. Jeff Kuss. The official was not authorized to discuss the matter publicly and spoke on condition of anonymity.
The rare Blues crash — the first in nearly a decade for the elite demonstration team, whose aviators are hand-picked to fly in tight formations and high-G force maneuvers — comes within hours of the crash of an Air Force F-16 Thunderbird after an Air Force Academy graduation flyby. The pilot ejected safely and is to be meet with President Obama.
It is also the third F/A-18 Navy crash in a week. (Note: I think I missed these stories)

Thunderbirds F-16 (Small Plane)
“A Thunderbird pilot was able to parachute to safety after aircraft issues caused his jet to go down in the Security-Widefield area Thursday afternoon.
The crash happened immediately following the annual Thunderbird performance at the Air Force Academy graduation. Witnesses say the plane landed in a field near the Powers and Fontaine intersection, across the street from First Baptist Church-Peaceful Valley. The field is owned by El Paso County.
The aircraft was on its final approach to the Colorado Springs Airport, according to Peterson Air Force Base spokesperson Jeff Bohn.
“The indication that we have that we received there radio-wise is that he did have an issue with the aircraft and that he was getting out of the aircraft,” Thunderbirds spokesperson Christopher Hammond said.”
“According to a bio on the Thunderbirds website, Turner is in his first year with the team, but has ample experience flying with the military. His bio states he has logged 1,200 flight hours as an Air Force pilot and more than 270 combat hours over Libya and Iraq.
Turner flies in the number six position when in formation, according to Hammond.”

South Alamo
Fixed-wing, single-engine aircraft or an experimental aircraft (FAA doesn’t know which?)
“Texas DPS and the Donna Fire Department responded to a plane that crashed near Alamo Friday morning.
The 98-year-old pilot crash landed on the yard of a home near Whalen Road between Ridge Road and Moore Road and U.S. 83 Business.
The man, identified as Dick Smith, stated that he lost power to the plane and could not restart the engine, according to Texas DPS troopers. The only damage that happened was to the tree that the plane landed on. Smith was able to walk away with just a minor cut on his hand. The crash is still under investigation.”
DPS Lt. Johnny Hernandez said preliminary information suggests engine failure. They’re waiting for FAA officials to arrive and determine the cause of the crash.
The pilot was identified as Dick Smith. He told CHANNEL 5 NEWS he built the plane himself and usually flies in the area. Smith said he was in the Navy and has been flying for 70 years.
The Federal Aviation Administration said the plane is a fixed-wing, single-engine aircraft or an experimental aircraft.
Smith said he made the call to the traffic control just before he crashed in a tree.
“Requirements of the FAA for a pilot’s license with endorsement of commercial pilot. So this is something that is very, very interesting to see a gentlemen or a person, still meeting the basic requirements physically for a commercial pilot’s license,” Emergency Management Coordinator George Garret said.
The FAA added there is no age limit to fly. It all depends on how the person feels and if they have a pilot license.
They are continuing their investigation how the plane crashed.

“Lucy Billen, a spokeswoman for TVP, confirmed a small aircraft had hit two cars, a Renault Megane which was unoccupied, and a second car which a woman in her sixties was sitting in.
The pilot and his passenger, both men in their 60s, were treated at the scene for minor injuries. The woman who was in the second car when the plane made contact with it was flown to John Radcliffe Hospital in Oxford with serious facial injuries.”
Chris Wheeler, social secretary for Brimpton Flying Club, said about 100 spectators were at the event to raise money for Thames Valley Air Ambulance.
“We are not yet aware of the cause but the aircraft had an accident on take-off and collided with two cars.
“Unfortunately one of the occupants was sitting on the ground between the two cars and she got hit by bits of the propellers which disintegrated on impact. ‘Although she is badly hurt we believe she is not in a critical condition.’ The pilot and his passenger were treated at the scene. An investigation by the Air Accident Investigation Branch is being carried out.”

Lake Thompson, South Dakota
(Small Plane)
“A pilot died Tuesday after crash landing into Lake Thompson. The passenger on board survived and is being treated at the De Smet hospital.
Crews investigated the crash all day Tuesday, but in order to know exactly what made the plane go down they had to dredge it from the lake, a process that took several hours.
Volunteers from area came out to help Kingsbury sheriff’s deputies pull the lane from the water. A boat spent several hours slowly towing it to shore so the craft wasn’t damaged any further. Once it was at the shoreline, crews worked to carefully move it out of the water. The plane’s passenger survived the crash. He told the sheriff the engine stalled before the crash, but crews want to know why.
“There’s a lot of factors you could look into but of course trying to decide what caused the crash or what caused the engine failure,” Sheriff Kevin Scotting said.
The passenger was rescued by a boater and taken to the De Smet hospital.
Sheriff Kevin Scotting says he is in good condition. The FAA is working with the Kingsbury Sheriff’s Office to investigate the cause of the crash.”

Single-Engine Homemade Aircraft (Small Plane)
“A pilot was seriously injured in a Wednesday morning plane crash near Batesville.
Indiana State Police said Terry Fruchtnicht, 50, of Osgood, Indiana, was the pilot of a small plane that crashed near the Batesville airport northwest of town just before noon.
The single-engine homemade aircraft experienced a mechanical problem shortly after takeoff, ISP said.
The plane then crashed into a heavily wooded area near Three Mile Road and Enochsburg Road. That’s at the northern end of the airport’s runway.
ISP spokesman Sgt. Stephen Wheeles said it took firefighters about 30 minutes to get the pilot out of the plane.
Police said Fruchtnicht was conscious and alert on scene. He was transported to UC Medical Center with serious injuries.
Fruchtnicht was the only person in the plane, Wheeles said.”
“As is typical in plane crashes, the Federal Aviation Administration responded to the scene to investigate the cause of the crash, spending approximately five hours on the scene Wednesday afternoon. That investigation is still ongoing, police said.
State police said the plane was made of wood with canvas material over the wings.”
“The cause of the crash is unclear.“

Houston, Texas (Hobby Airport)
Cirrus single-engine SR-20 (Small Plane)
Three people were killed when a small plane crashed near Hobby Airport on a flight from Norman, Okla. Thursday afternoon. It’s a miracle no one on the ground was hurt, investigators say.
The Cirrus single-engine SR-20 crashed in the parking lot of an Ace Hardware Store just after 1 p.m. It narrowly missed the store full of people.
The plane went down a few yards from a large propane tank but there was no fire, according to HFD. It also missed power lines in the area.
Lathson said the pilot was “too high” on her first two attempts to land and was told to “go around.”
In a recording from the Air Traffic Control tower just before the crash, a controller is heard telling the pilot, “Ma’am, ma’am, straighten up! Straighten up!”
“After executing a go-around maneuver, the aircraft was seen to descend suddenly nose-first into the parking lot,” Lathson said.
The plane apparently stalled before it nosedived, according to Lathson.
Audio from the Air Traffic Control tower indicates Dana Gray was trying to land between two 737s when she became confused.

Three killed in North Dakota plane crash - Piper PA 28-140 that crashed into May Lake Wishek Airport ND
“Authorities have identified the three people killed when a small plane piloted by a 20-year-old crashed into a North Dakota lake Thursday - the same day another collision occurred in Texas.
Colbie Fandrich, 20, was flying his aunt, Christine Fandrich, 38, and her son Aaron Nordstrom, 10, when their plane crashed into May Lake, KFGO reported.
The announcement came on the same day as another small plane crash in Los Angeles and the day before a crash in Tennessee brought the number of such accidents in the past week up to four - and the body count to ten.”
“Fandrich - who’d had his flying license for a year before the crash - lost control of the plane due to complications during takeoff from a nearby Wishek Municipal Airport Thursday afternoon.
They had intended to fly to Bismarck. The four-seater Piper PA-28 plane was being removed from the lake Friday, police said.”

Collegedale, Tennessee
1964 Mooney M20E (Small Plane)
“A handful of local pilots talked quietly with each other outside the Collegedale Municipal Airport office Sunday afternoon as officials from the Federal Aviation Administration and the National Transportation Safety Board inspected a grim scene several hundred yards away.
The airport reopened to flights Sunday, but a somber mood remained as officials investigated the Saturday crash that killed the pilot’s mother and son. (UPDATE: The pilot passed away on June 12th)
Collegedale police said Suzanne Silver and her juvenile grandson died from injuries they sustained when the 1964 Mooney M20E flown by Todd Silver of Fort Lauderdale, Fla., crashed along the 5,000-foot runway on a scorching day in the rolling hills near the Bradley County line.
Todd Silver and his juvenile daughter were airlifted to Erlanger hospital from the scene. Todd Silver runs an airplane canopy business called Todd’s Canopies, according to his Facebook page.
NTSB spokesman Eric Weiss said a preliminary report on the crash will be released in seven to 10 days, while the full investigation will take a year.
Meanwhile, others with experience flying out of Collegedale were left to wonder what caused the tragedy.”
“It’s unclear if Todd Silver had ever landed at Collegedale Municipal Airport. Speculation abounded among aviation veterans Sunday about how weather conditions such as heat and wind could have adversely affected an otherwise routine landing.
“Yesterday was a very hot day, a very humid day. And those two factors, along with higher density altitude, really all three of those play into a degradation of aircraft performance,” said veteran pilot Morty Lloyd, who lives next to the airport.
“But a general aviation aircraft is very, very safe” he said. “Most general aviation accidents happen because of human error. I’m not saying this was human error. NTSB will have to make that determination.”
But all agreed it was a tragedy and said, generally speaking, flying is a safe means of transportation.”

Bears Glen area of Keystone Lake, Cleveland, Oklahoma
(Small Plane)
“Authorities says the pilot of a small airplane made a hard landing in a field near Keystone Lake west of Sand Springs Saturday morning.
The Oklahoma Highway Patrol says the small plane went down in an area east of Cleveland and north of Bears Glen at about 9:30 a.m.
Firefighters from the Peninsula Fire Department responded to the scene.
Troopers said the plane’s 57-year-old pilot was taken to a Tulsa hospital to be treated for a head injury, but is expected to be OK. The OHP says it appears the plane experienced mechanical problems and the pilot was forced to land in the field.
They said the nose of the single engine plane was destroyed.
People in the community say the pilot is very familiar with the area, which is why they say he chose this field to land in, in order to avoid the trees.
The airplane barely missed the tops of trees before smashing the nose into the field below.
“When I woke up, my wife said that she’d saw Rich fly over this morning and that the plane really sounded bad,” retired U.S. Navy pilot Art Long said. “The engine was sputtering and she said it was just right over tree top level.””
“The FAA will look into the cause of the crash and will release information when they get more details.”
